Education, Health & Care Plans / Annual Reviews

  • Oakley School works collaboratively with the Local Authority within the district to meet the needs of children and young people who have an Education, Health & Care Plan (EHCP).  However, it is the Local Authority who has the statutory responsibility for arranging provision for children and young people with Education, Health & Care Plans (EHCPs).  

 

  • An Annual Review meeting is held with parents, carers, members of staff, support agencies and pupils themselves each year to discuss whether the school is continuing to meet the needs of the child or young person, detailed in the EHCP and the individual targets or outcomes set for that child.  From Year 9 onwards, this is combined with a Transitional Review which explores the opportunities and pathways for the child as s/he progresses towards adult life, in preparation for leaving Oakley School.

 

  • Children and young people have Outcomes and Targets set during each year, in consultation with parents based on their Education, Health & Care Plan.  Children and young people work towards achieving these targets on a daily basis, and the child’s class teacher or tutor, aided by the learning support assistant/s, will be assessing and assisting in the progress together with input from the therapists. The targets are reviewed and discussed with parents at Parent Consultation Evenings.  Please see the presentation below for more information about setting Outcomes and Targets.

 

  • End of Year Reports of educational progress are sent home in July.  Progress and achievements are discussed in detail at the Annual Review of the EHCP.  As a result, parents, teachers and support staff agree priorities to focus on for the next year.  There are also two parent consultation evenings each year.
